Event Participant Waiver and Release
In consideration of your acceptance of my entry, I hereby certify, agree and acknowledge:
I understand and agree that I must report to the above Event sufficiently fit and healthy enough with sufficient experience and equipment to safely participate in the Event, and I will conduct myself so as to protect myself and other participants at the Event. I am aware that the risks associated with the Event include the possibility of personal injury, property damage, or death.
I am aware of the above risks and responsibilities and agree that my voluntary participation in the Event is at my own risk. On behalf of myself and anyone who might sue on my behalf, I hereby irrevocably waive any and all claims against the Dana Point Yacht Club and the County of Orange, and their officers, directors, members, employees, agents, volunteers, contractors, insurers, successors and assigns, for damage, injury, accident, illness, or death, including any provision of medical first aid, which may relate to the Event.
I understand that my image may be recorded at the Event, and I hereby give my permission to use my images with or without my name for any promotional purpose of the Dana Point Yacht Club.
I certify that I am 18 years of age or older in order to participate in this Event.
